Enroll in the US Family Health Plan 

Our enrollment process only takes about 15 minutes—make sure your DEERS information is up-to-date and let's get started.

Please note: TRICARE Young Adult Prime® has a different enrollment process and application than the US Family Health Plan. Information about enrolling in TRICARE Young Adult can be found further below. 

First, confirm your eligibility

  • DEERS enrollment: Are you (or your sponsor) registered in the *Defense Enrollment Eligibility Reporting System?
  • Enrollment window: Are you applying during Open Season (Nov 1 – Dec 15), within 90 days of a Qualifying Life Event (e.g. PCS, marriage, retirement), or within 90 days of first becoming eligible?
  • Service area: You must live within our Martin’s Point US Family Health Plan region (ME, NH, VT, upstate & western NY, and parts of PA & OH).

The following individuals may be eligible to enroll in the US Family Health Plan:

  • Active-duty family members, including spouses and unmarried dependent children. Dependent children are eligible until their 21st birthday or, if they are a full-time student, their 23rd birthday
  • Military retirees from the active component, their spouses, and unmarried dependent children
  • Military retirees from the reserve component upon reaching age 60, their spouses, and unmarried dependent children
  • Family member survivors of a deceased active-duty military member or military retiree
  • Former spouses of military sponsors who meet TRICARE eligibility rules

If you meet the eligibility requirements above, you may be able to enroll if one of the following applies:

  • You’ve experienced a Qualifying Life Event (QLE) in the last 90 days. Some examples include marriage, divorce, or the birth of a child. View the full list of QLE’s.
  • TRICARE Open Season runs annually from the Monday of the second full week in November for 30 days. See TRICARE Open Season.
    If you are coming from another TRICARE Prime program, you may enroll anytime during the year.

Primary Care Manager Name & location of your preferred PCP(s). If unknown, we’ll assign one—you can change it online anytime.
Qualifying Event Docs (if not Open Season) e.g. PCS orders, marriage certificate, retirement paperwork
