For applicable in-network benefits, US Family Health Plan members may choose to receive care from over 25,000 providers across our service area (including all the major health care systems) who participate in our US Family Health Plan network.
Our service area includes Maine, New Hampshire, Vermont, upstate New York, northern and western Pennsylvania, and northeast and central Ohio.
In-Person Urgent Care Facilities
Urgent care facilities listed in this directory are in-network. However, the US Family Health Plan allows you to visit any urgent care facility, no matter where you are.
