Know which plan you want to enroll in. Use the plan comparison tool to find the best fit for your needs.
Your Medicare Number and your Part A and Part B start dates found on your Medicare card.
Information about any other health coverage you have, including policy and group numbers.
Dates for any changes that take effect, like moving to a long-term care facility.
